Our service is providing a recommendation/referral to you to a telemedicine provider and/or service, which may or may not include collecting and sharing any and all necessary information needed for the telemedicine provider’s service ONLY.  Clients may still have to pay for the telemedicine provider’s consultation as well as any additional fees and costs associated with their care. The resource consultation and/or service fee paid to Attest Medical is NOT refundable. By continuing, you authorize Attest Medical to release your information and/or a copy of this agreement/statement to your financial institution if you later dispute this charge.
